Haven at Home Inc. is a home healthcare agency based in Toronto, Ontario, providing in-home physiotherapy, personal support, nursing, and related care services. Our registered office is at 1033 Bay St., Suite 317, Toronto, Ontario M5S 3A5.

Home care services provided by Haven at Home are governed by a separate Service Agreement entered into between the client (or their authorized representative) and Haven at Home. That agreement sets out the specific terms for:

• The type and scope of care to be provided.
• Scheduling, frequency, and duration of visits.
• Fees, payment terms, and cancellation policies.
• Responsibilities of both the client and Haven at Home.

Submitting a booking request or inquiry through the Website does not create a binding agreement for services. Services begin only after a Service Agreement has been signed and an initial assessment has been completed.

5. Booking and inquiries

When you submit a form on the Website or contact us by phone or email, you agree to provide accurate and complete information. We will use the information you provide to respond to your inquiry and, if applicable, to schedule an assessment or begin the intake process.

We aim to respond to all inquiries within one (1) business day. Response times may vary depending on volume.

6. Intellectual property

All content on the Website — including text, images, graphics, logos, icons, and design — is the property of Haven at Home Inc. or its licensors and is protected by Canadian copyright and trademark laws.
