What the assessment covers

Balance Assessment

We use the Berg Balance Scale — the clinical gold standard — to measure your loved one's balance and falls risk. We also assess gait (how they walk), their stride pattern, and any compensatory habits that signal risk.

Strength & Mobility Testing

Weak legs and poor core stability are primary fall causes. We assess lower body strength, functional mobility, and the ability to recover from balance disruptions — all in your home environment.

Medication Review

Around 40% of falls in seniors are medication-related. Certain blood pressure drugs, sedatives, antihistamines, and diuretics significantly increase falls risk. We review the full medication list and flag any combinations or timing issues worth discussing with the prescribing physician.

Home Hazard Assessment

A PSW walks every room of the home — bathroom, bedroom, kitchen, stairs, entryways. Loose rugs. Poor lighting. Furniture in tight paths. No grab bars where they're needed. We document every hazard and prioritize what to fix first.

Your Written Plan

You don't leave with a vague conversation. You leave with a written Stay Safe at Home Plan — specific recommendations, prioritized by risk level, with clear next steps. We can share this with your GP, specialist, or anyone else on your care team.

THE PROCESS

What to expect on assessment day

We arrive together

A physiotherapist and a PSW come to your home at a scheduled time. Expect 60–90 minutes for a thorough assessment.

We work through the assessment

The physiotherapist leads the clinical balance and mobility testing while the PSW conducts the home walkthrough. We explain what we're doing as we go.

We debrief with you

Before we leave, we walk you through the key findings — what we found, what it means, and what we recommend.

4

You receive your written plan

Within 24–48 hours, you receive a written Stay Safe at Home Plan with all findings and prioritized recommendations.
