Diabetes Care

Consistent daily support for a daily condition

Managing diabetes is a daily commitment — and consistent support at home makes all the difference. Blood sugar monitoring, insulin timing, diabetic-friendly meals, and foot care are easier and safer when someone is there to help. Diabetes also significantly increases fall risk: peripheral neuropathy affects balance, hypoglycemia causes dizziness and disorientation, and complications like vision loss make navigation dangerous. We work with your loved one's care team to keep them stable — and safe on their feet.

Why families choose Haven at Home

PSW vs nursing: we match the right level of care — daily support and meal prep from a PSW, or insulin and clinical monitoring from our nursing team when needed.

We watch for the warning signs that predict both health crises and falls: hypoglycemia episodes, blood pressure changes on standing, neuropathy progression, and slow-healing wounds.

We keep you in the loop with clear updates so you know how your loved one is doing — including their fall risk — without having to be there every day.

Signs you might need this

If any of these sound familiar, we can help.

Is your loved one managing Type 1 or Type 2 diabetes?
Do they need help with blood sugar monitoring or insulin administration?
Are they struggling with diabetes-related diet and meal planning?
Have they had diabetic foot issues or skin complications?
Do they need medication reminders and refill coordination?
Are you worried about hypoglycemia or missed doses when they're alone?

Common questions

Can you help with diabetic diet and meal planning, or just medication?+

Both. Our caregivers can prepare diabetic-friendly meals, monitor carbohydrate intake, and ensure proper meal timing. For complex dietary needs, we can coordinate with dietitians.

My parent's blood sugar swings a lot. What do you do about that?+

We track patterns, ensure medication timing and dosing are followed, support proper nutrition, and communicate trends to the medical team. Unstable blood sugar often has identifiable causes that consistent monitoring can help address.

Do you need a nurse for insulin, or can a PSW help?+

Insulin administration requires our nursing team. A PSW can provide reminders, meal support, foot checks, and blood sugar logging — and we can combine PSW visits with nursing visits for insulin when needed.

What if they have a diabetic foot ulcer?+

We coordinate with their doctor and wound care team. Our nurses can perform dressing changes and monitor healing. Early, consistent care reduces the risk of serious complications.

How do you communicate with our endocrinologist?+

With your permission, we share logs, trends, and concerns. Many endocrinologists appreciate having eyes on their patients between appointments — it helps with medication adjustments and early intervention.
